Driving Directions
From Walnut Creek
Take Mount Diablo Boulevard west for approximately three miles. Our campus is on the right between Brown Avenue and Lafayette Circle, a quarter mile past the Lafayette BART station. Allow about 10 minutes from downtown Walnut Creek.
From Oakland and the East Bay flatlands
Take CA-24 East through the Caldecott Tunnel for about 12 miles. Exit at Central Lafayette / Pleasant Hill Road and head north. Turn right onto Mount Diablo Boulevard; the campus is approximately a third of a mile east on the right. Allow 25 to 35 minutes from downtown Oakland in standard traffic.
From San Francisco
Take I-80 East across the Bay Bridge to CA-24 East through the Caldecott Tunnel. Follow the Oakland directions above. Allow approximately 45 to 60 minutes from downtown San Francisco depending on traffic and tunnel congestion.
From the Tri-Valley
Take I-680 North to CA-24 West. Exit at Central Lafayette / Pleasant Hill Road, follow the Oakland directions. Allow about 25 minutes from Pleasanton.
BART access
The Lafayette BART station is a five-minute walk from our front entrance. The station is served by the Yellow Line.
Arrival notes
On-site parking is free and clearly marked for admissions arrivals. If you are driving someone in crisis, pull directly into the admissions semicircle and a nurse will meet the vehicle. The address is 3587 Mount Diablo Blvd, Lafayette, CA 94549.
Service Area
RBHN Rehab primarily serves patients across the East Bay and the broader San Francisco Bay Area - Lafayette, Walnut Creek, Pleasant Hill, Concord, Orinda, Moraga, Alamo, Danville, San Ramon, Berkeley, Oakland, Albany, Emeryville, and surrounding communities. Our Mount Diablo Boulevard location was chosen for proximity to BART, the I-680 and CA-24 corridors, and the East Bay hospital network.
Hospital-to-hospital transfers are coordinated directly with referring clinicians at John Muir Health (Walnut Creek and Concord), Kaiser Walnut Creek, Sutter Lafayette, Stanford ValleyCare, Highland Hospital, Alta Bates Summit, and the broader Sutter, Kaiser, and Dignity Health networks. Transfers typically complete within a few hours of the initial call.
Patients traveling from outside the Bay Area are welcome. Our admissions team coordinates pickup from OAK, SFO, and SJC airports and handles pre-admission logistics so the first day is about arrival, not navigation.
